Diagnosis of sleep disorders

To diagnose OSAHS, our doctor will conduct a questionnaire on a sleepiness scale, or you can provide him with “homework” using our table for the primary diagnosis of apnea.

Next, you will be given an overnight cardiorespiratory test, or overnight pulse oximetry test. All studies are painless, non-invasive and performed at home. In diagnosing breathing disorders during sleep, we use expert-class devices - NONIN Wrist Ox2 (USA) for overnight pulse oximetry and Apnea Link Plus (Australia, Germany) for cardiorespiratory tests.

During the study, your heart rate, saturation (oxygen saturation in the blood), respiratory rate and other important parameters will be recorded. Next, the doctor deciphers the study and issues a conclusion and protocol of the study.

Treatment of snoring and apnea

When prescribing treatment, we use diagnostic criteria, algorithms and recommendations of the European Sleep and Research Society (ESRS), American Academy of Sleep Medicine (AASM), and the Russian Society of Somnologists (ROS).

If the study reveals snoring or respiratory arrest that is not accompanied by hypoxia and cardiac disorders, we will recommend treatment by otolaryngologists and dentists.

If problems require non-invasive ventilation during sleep, we will select modern and reliable devices for CPAP or BPAP therapy.

This catalog contains somnology centers (sleep medicine centers) operating under the auspices of the Russian Society of Somnologists, the only professional public organization of specialists in the field of sleep medicine in Russia. Currently, the Russian Society of Somnologists is carrying out voluntary accreditation of somnology centers of the Russian Federation in accordance with international standards in the field of sleep medicine. A full-service inpatient sleep center (FSSSC) is a medical unit/center that diagnoses and treats a wide range of sleep disorders and has the ability to conduct overnight polysomnographic studies under the supervision of staff, including online monitoring and video surveillance, and conduct CPAP and BIPAP therapy under such observation.

Fully functional mobile sleep center (FMSSC) is a medical unit/center that has the ability to conduct overnight polysomnographic studies without personnel supervision, including online monitoring and video surveillance, and conduct CPAP therapy without such supervision.

Respiratory Somnology Center (RSC) is a medical unit/center that has the ability to conduct overnight polygraphic studies (cardiorespiratory and/or respiratory monitoring) and conduct CPAP therapy without personnel supervision.

For somnology centers that have not yet been accredited, the same terminology is retained: inpatient - having dedicated wards/beds for conducting somnological studies; outpatient – ​​sleep studies are carried out only outside the center.

CPAP therapy

We prescribe CPAP therapy only on the basis of the tests performed and the absolute indications for this type of treatment (nocturnal hypoxia, risk of death during sleep).

The most effective treatment for sleep apnea worldwide is a device that creates continuous positive pressure in the airways to prevent them from collapsing. Treatment with this device is called CPAP therapy.

Patients who receive CPAP therapy wear a mask that is connected to a special ventilator throughout the night.

If you are diagnosed with OSA accompanied by hypoxia and heart rate abnormalities (tachy or bradycardia), then the only treatment option is CPAP (CPAP) therapy.

Polysomnography is the gold standard for diagnosing sleep disorders such as sleep apnea or narcolepsy. Polysomnography allows you to assess how the body's systems function during sleep and identify possible pathologies and their cause. The examination takes place in a clinic/somnology laboratory. How to prepare for a polysomnographic examination the day before? • Your day should proceed as usual. • Don't sleep during the day. • Avoid drinking caffeinated drinks after lunch. • Do not use hair styling products: since the electrodes are applied to the surface of the head, the signal recording may be incorrect due to hairspray or gel. • Check with your doctor to see if you need to take your medications on the day of the test.

What to take with you?

In most cases, you need to be in the lab around eight o'clock yesterday. Take comfortable pajamas and everything you need to perform your usual activities before bed (toothbrush and toothpaste, cream, book). If necessary, take medications (check with your doctor and laboratory staff).

How will you prepare for the study?

A laboratory worker will conduct a survey about how you usually sleep, and you may be given a short questionnaire to fill out. Then you can change your clothes and get ready for bed. After this, the technologist will apply electrodes and connect the necessary sensors - they are either glued to the skin or attached with belts. If you are allergic to any adhesives, tell the technologist. Ask the specialist any questions you have about the study that concern you. Let us know if you would like to be woken up at a specific time. Once the sensors are installed, you can read or watch TV until you fall asleep. If necessary, you can get up at night to go to the toilet. There is a camera installed in the room - while you sleep, the technologist will watch you and will be able to come up at any time if you need help or some sensor becomes detached.

What if I can't sleep?

It seems very difficult to fall asleep with many sensors attached, but most patients manage to sleep long enough to make a diagnosis. In some cases, you may be prescribed sleeping pills.

What is a CPAP test?

In most cases, the doctor will refer the patient for a polysomnographic examination in order to identify a disease such as obstructive sleep apnea. Obstructive sleep apnea is when breathing stops during sleep when the airways become blocked and the flow of air into the lungs stops. You can read more about this syndrome in this article. The most effective method of treating obstructive apnea today is CPAP therapy. The closure of the pharyngeal tissues is prevented by the method of introducing air into the respiratory tract. There are several types of CPAP devices; in addition, each patient needs to select a mask and treatment pressure settings.

The selection of the device can begin during an overnight polysomnographic study. This design is called a split-night study. During the first half of the night, the technologist assesses how you breathe while you sleep. He then puts a mask on you that is connected to a CPAP machine. The rest of the night you sleep using the device. By assessing breathing parameters, the specialist selects the optimal treatment settings for you.

What happens after the examination?

In the morning, the technologist will remove all electrodes and sensors, and you can go home. The data recorded during the examination will be studied and interpreted by a specialist. This may take from several days to a week. After sharing the results with the doctor who referred you for polysomnography, he or she will discuss the results and future treatment strategy with you.
